There are two sections on the Insurapply website where Insurapply will ask you for your personal information (PI) and Health Information (HI)
Note: If you are just viewing the Insurapply website, there is no requirement to provide PI or HI.
If you would like to view price, premium, cost or rates on the Insurapply quoting engine, PI will be required. The quoting engine is a software program that requires your name, date of birth, zip code, and tobacco use or not. We collect email address on the quoting engine, but the phone number is optional.
If after viewing the initial quote information, you wish to proceed and apply for life insurance coverage, additional PI and HI will be required to complete the application process. This may include but is not limited to:
Once your application has been submitted to the life insurance company for review (Underwriting), the insurance company, not Insurapply, will begin to access information about you that may include but is not limited to your: credit report, MIB, prescription database, MVR, and consumer report. If medical records from your physician are required, Insurapply with your consent and signed HIPPA authorization, will be able to obtain these documents.
Insurapply: Use of your PI/HI
Insurapply will use your PI and HI for the quoting engine and for processing your life insurance application. As your life insurance broker, and acting as your agent, this information is for internal business purposes and used to provide to the life insurance carriers directly, required information for underwriting requirements.
